1) There is generally poor assessment of risk factors
2) AKI is a global health care issue that is on the increase
3) Complications missed (13%), avoidable (17%) or badly managed (22%)
4) The majority of cases arise and/or are managed in the community or across all specialities within secondary care (Selby, et al, 2012)
5) 22 patients died with a primary diagnosis of post-admission AKI which was predictable and avoidable
6) Estimated additional cost to the NHS of £500m (NHS Kidney Care) per annum (projected losses in the NHS this year are estimated to be £700m)
